Although disease progression may be one of the contributors to ill-health in clients, some patients often experience negative health outcomes as a result of the stigma associated with a disease. The problem becomes even worse when vulnerable populations are excluded from the community due to disease. The advanced practice nurse has a significant role to play in helping patients to eliminate the stigma associated with a disease. The healthcare professional can function as an advocate for vulnerable populations in various ways based on a specific problem that a person is facing at any given time (Gronhilm, Henderson, Deb, & Thornicroft, 2017).
The type of assistance that the advanced practice nurse will provide to vulnerable populations that face stigma due to various health conditions must aim to achieve the Health People 2020 objectives (ODPHP, 2017). For instance, the nurse can help a 24-year old pregnant woman who is currently homeless and unmarried in two major ways. First, the practitioner can organize how the patient can access appropriate prenatal and preconception healthcare from available healthcare facilities. Second, the healthcare professional can assist the patient in establishing contact with shelter facilities that can provide her housing needs. In this manner, a healthy community with reduced rates of maternal demises, infant deaths, and stable housing will be created (ODPHP, 2017).
Some of the factors that define a healthy community are an improvement in adolescent’ health and a lack of issues related to substance abuse. In this respect, the advanced practice nurse can assist a 16-year-old girl who has run away from her foster home and who has an IV drug habit by helping her to establish a connection with a counselor. According to ODPHP (2017), contrary behavior patterns are some of the causes of poor health among adolescents. Therefore, in the given scenario, the counselor will select appropriate interventions to influence the client to develop positive behaviors (ODPHP, 2017).
